Pharmacy, School Partner

FRUITLAND — Apple Discount Drugs, a locally owned Health Mart pharmacy, has announced its participation in a hypertension management pilot program initiated by the University of Maryland. The University of Maryland School of Pharmacy P3 (Patients, Pharmacists, Partnership) Program is a patient-centered collaboration of patients, primary care providers and specially trained P3 pharmacists.

The program incorporates comprehensive medication therapy management, chronic diseases state management and the development of patient self-management skills in order to improve clinical outcomes, increase adherence to current standards of care and reduce overall healthcare costs. The program has been in existence for diabetes management but Apple is the first pharmacy in Maryland to pilot the same initiative for hypertension.

"It made sense to pilot the P3 program with Apple Discount Drugs because the pharmacy reaches such a broad scope of patients across the Eastern Shore of Maryland," said Dawn Shojai, Pharm D, assistant director of the University of Maryland P3 Program. "We also considered the capabilities of the pharmacy we selected in regards to the coverage of management for the program and Apple has gone above and beyond our expectations, growing the number of counseled patients from 40 to more than 56 over a six-month time period."

Apple’s Geoff Twigg, ambulatory care pharmacist, Sara Mae Brown, Pharm D, and Katy Miller, R.Ph. are specially trained pharmacists who provide step-by-step guidance in medication adherence, lifestyle changes, and self-care skills to 65 Apple Discount Drugs customers.

"The end goal of the P3 program is to develop dynamic partnerships between patients, primary care physicians and pharmacists that will ultimately provide a growing number of employers’ assistance in improving their employees’ health management skills," said Geoff Twigg, Apple Discount Drugs ambulatory care pharmacist. "Working with the patients is proving to be very successful. We are identifying healthcare issues they are facing on a daily basis and providing lifestyle solutions to help overcome the challenges."
